We welcome patients of all ages! We are a full service dental practice, offering services ranging from basic preventative care to full mouth rehabilitation. We welcome you to our office, and thank you for choosing us for your health needs. While our specialty is the orofacial (the mouth and face) area, our goal is for your complete body health. Our mission is to build a long-term relationship with our patients and provide top quality care in a warm and friendly atmosphere. Our team of experienced health professionals offers caring, comprehensive orofacial care. We are advocates for prevention. Your role in maintaining good orofacial health is important, and we are here to support you every step of the way. We have a strong tradition of care, and service, and we value your comfort. Your First Visit

Your first visit will consist of a detailed comprehensive orofacial examination to help us determine the baseline status of your current orofacial health. It is very important to establish your baseline, identify your risk factor(s) for disease, and implement any needed changes for optimal health at your initial visit. This will allow us to better track changes in your health in the years to come.

This initial comprehensive examination will allow us to establish your baseline, identify your risk factors and current needs, as well as provide us with the information we need to develop your individualized treatment plan and preventative recommendations. We hope that you are a patient at our office for generations, and we do not take it lightly that you have chosen us for your care.

Patient Forms

We are a paperless office, and you will be completing the patient registration portion on a computer. Please plan on arriving 10-15 minutes prior to your appointment to complete the registration portion. We will provide assistance with completion should you need it. Please print the following forms, review & sign, and bring them to your first appointment.

Apply for Care Credit
We will discuss your treatment recommendations, the cost of your treatment, and your payment options before you begin any treatment so that you can make the best informed decision regarding your health.

Our practice staff will provide you with an estimate of dental coverage, and assist you with the insurance claims and paperwork. Please understand that we strive to estimate your benefits as closely as possible, however, when a third party payor is involved fees cannot be exact.

Unless specific written arrangements have been made in advance with our office, we ask that you pay your portion of the fee on the day the service is provided.
Please be advised that it is also your responsibility to pay any balances not paid by your insurance company. We will contact you via telephone and/or bill you for any unpaid balances, the balances are due upon notification. Our practice accepts most major credit cards and we work with most PPO dental insurance plans.

We are partnered with Care Credit to better assist you with payment flexibility. We will assist you in applying for Care Credit. Care Credit allows you to pay for and receive needed services right away, and then make convenient monthly payments.

Insurance FAQs

Do you take my insurance?

We work with most PPO dental plans. A PPO plan allows you to choose your own provider.

Are you in network?

We are “in network” with several PPO plans. If in doubt, please check with your plan to minimize your out of pocket costs. We do not have access to copies of patients' benefit booklets, we rely on the information you and your insurance company provide us. For that reason, it is recommended that you personally contact your insurance company to clarify your benefits when making decisions regarding your care.

Patient Notice

We do not treat you according to what insurance plan you have, or based on the limitations set forth by your plan. Doing so would be unethical, and not in the best interest for your longterm health benefit.

While we will help you optimize your insurance benefits, we do not formulate your treatment plan based on what your insurance company covers, we based it on what we consider the highest quality of care for you. We will always recommend what is in your best interest, and we are here to assist you when you need us.

We are here to provide you the best quality care possible, provide you with the best options for that quality care, and to assist you in making that care possible for your longterm benefit. We that in mind, we will support you in achieving your optimal health.
